Southwest Adult Day Services is a licensed, not-for-profit, multi-ethnic, community-based program for elderly and disabled adults. We provide nursing care, restorative therapies, social services, and recreational activities. Our mission is to help older, frail adults, as well as younger, functionally impaired adults, remain independent as long as possible. We accomplish this mission by improving and maintaining each participant’s health and functional abilities to their optimum level by delaying or preventing premature placement in a skilled nursing facility or other institution and by enhancing the quality of life for each participant and their caregiver.

Transportation is available to and from Adult Day Services, if needed. Our service area includes primarily Santa Rosa and Windsor. Transportation for participants from outlying areas such as Sebastopol, Forestville, Rohnert Park, Oakmont, and Healdsburg can also be arranged. Otherwise, caregivers provide transportation.

Insurance & Fees:
Everyone is welcome at Santa Rosa Community Health Centers, even if you are uninsured or can’t afford to pay.
Our Adult Day Services program is Medi-Cal certified. That means the Medi-Cal determines if patients are eligible for payment for services and if there will be a share of cost for services for each patient. In order to find out if you qualify for Medi-Cal you must apply at the local county offices.
We also offer a sliding fee scale for private pay clients that ranges from $80-142 per day. The amount is based on income level compared to the federal poverty guidelines. Those accessing the program through private insurance
are charged a daily rate of $150.
