Adult Day Services is a six hour program which is open from 10:00am to 2:00pm for frail, elderly or disabled adults. The program includes transportation to and from our beautiful facility, as well as snacks, activities, and a nutritious lunch. This program is provided to adults with potentially debilitating conditions including older, frail adults, as well as younger, functionally impaired adults. The goal of Adult Day Services is to help those we serve to remain independent as long as possible.

Insurance & Fees:
Everyone is welcome at Santa Rosa Community Health Centers, even if you are uninsured or can’t afford to pay.
Fees: The cost of services may be covered by Medi-Cal. Call us at 707-573-4522 to find out if you are eligible. Not covered or uninsured? Not a problem. We offer a sliding fee scale. The exact fee is calculated based on your family size and monthly income.
