Outreach & Health Education

The mission of the Santa Rosa Community Health Centers’ (SRCHC) Outreach Team is to let the community know about SRCHC programs and services. The Outreach Team members spend the majority of their time in the community at local homeless shelters, at health fairs, in street outreach at parks, at transit centers, and at food distribution centers.  They provide health education in local schools and at the Santa Rosa Junior College; they attend public meetings and local events to educate attendees on how to access SRCHC’s services. SRCHC’s Outreach and Education Program is funded primarily through family planning grants.

Locations Where Outreach Services are Provided:

Health education is provided in classrooms at local middle schools and high schools.   In addition, SRCHC has partnerships with numerous local community agencies  in which we provide regular health education for their clients and help their clients make any needed appointments at SRCHC facilities.  Current partnering agencies include Women’s Recovery Services, Sloan House, and El Centro Laboral de Graton.

Outreach staff members also work within the SRCHC clinics as Health Educators for the Family Planning Program.
