Primary Care

Santa Rosa Community Health Centers provides comprehensive health care for the individual and family across all ages, genders, and medical problems. Patients have an ongoing, continuous relationship with a physician, nurse practitioner, or physician assistant as their “Primary Care Provider” who is a part of a health care team.  Care is provided in the context of the family and the community, emphasizing disease prevention and health promotion. Our primary care services are provided to patients of all ages.

Our services include:

  • Preventive care
  • Screening for early detection of disease
  • Diagnosis and treatment of illness
  • Referral to speciality care

Insurance & Fees:

Everyone is welcome at Santa Rosa Community Health Centers, even if you are uninsured or can’t afford to pay.

We accept: Medi-Cal, Medicare, CMSP, Healthy Families, FamilyPACT, Gateway/CHDP, and offer low cost fees if you are uninsured.  Families needing help to apply for low-income insurance can call 707-547-2222 (Southwest) or 707-303-3600, ext. 504, (Vista) for an appointment with our Certified Application Assistants.

No medical insurance?  Not a problem.  If you are uninsured, we offer a sliding-fee scale for office visits; the exact fee is calculated based on your family size and monthly income.
