To schedule an appointment, call the health center location where you receive care. Every patient may select their own personal physician, who along with your care team will be your medical “provider” for all your planned visits.
Important note: We can only schedule same-day or urgent appointments for established patients. Please don’t wait until you are sick to call for an appointment; call and make a “new patient appointment” to ensure that you can be seen promptly when you need medical care.
